The most common fault is an obstruction, quite often caused by the wiring from any of the 3/4 reels not being routed properly, and catching on this reel. Anything interfering with the spinning of the reels will trip an alarm. Other silly causes can be a coin dropped inside the reel throwing it off balance. Also check the connection to the board, sometimes unpluging it and reconnecting it can resolve the issue.
Failing these simple tests, swap the reel with another one, and see if the fault moves to the "new position". If it does, then the fault is obviously with the reel itself, and best thing to do is replace it.
What machine is it off, as I may have a spare ? drop me a pm.
