Does this really happen? I can't say I've encountered one personally. A machine did reset on me once while invincible, thankfully only a cat D.

I guess if you miss skill continue. Or if it gives you a hint like a certain number next and you gamble the other way. Thats all I can think of.

My sketchy brain seems to remember an IM that would give skill continue off a ?, if missed you lost, maybe Big Cheese or Top Dog? There has deffo been games you can lose on an IM.

Years ago i hit lose on a skill stop for a joke on a £5 machine that actually did kill me, but never not self inflicted.

Money Spinner is misleading on an IM.
"Silent" IMs on Games Media can sometimes kill, though they're not the proper IMs.
Reds and vivids do kill but let you back on lol
Vindaloot can kill
LOTR return of the king has killed when going against the gamble sometimes

On some of those late 90s 'crests, the problem on the IM board was 'Collect Prize' being given from the mystery.

So not really a 'lose', but it sure as hell felt like it.

Seem to remember something about Squid Vicious Kicking you out on a ? by offering cash out,so i suppose it could happen on any mc with this option on ?.....
